Conners RTC Referral Process

To make a referral, please contact the Director of DSS Programs at 884-3802. When making a referral the following information is helpful: Social Assessment, Psychiatric Evaluation, Psychological Assessment, Educational History (IEP), and Medical History including immunization record. Youth served by the program must be approved by the local department of social services who assumes custody while the child is in placement. Conners Children’s Center staff will provide assistance to the referring party to work along with the child’s local department of social services.

Morey House Group Home Referral Process
